We are building a culturally-focused, technology-enabled healthcare organization that integrates comprehensive medical care and social support with a high-touch, interdisciplinary approach. Building upon a proven Home and community based services model, we are embracing the unique cultural and healthcare needs of the communities we serve and empowering our staff with thoughtfully designed technology to scale the highest quality care to many more older adults.
