NeighborHub Health (NHH) serves individuals experiencing homelessness or living with HIV/AIDS in the Tri-State area, as it has since 1986. We offer quality medical, behavioral and case management services. As a Federally Qualified Health Center (FQHC), NHH has earned the distinguished Patient Centered Medical Home (PCMH) Level 3 seal, the highest level possible, proving we go the extra mile to ensure patients receive outstanding care. NHH runs several integrated medical and behavioral care clinics in the area, making it one of the few local organizations that provide this type of integrated care...
