Carol Silver Elliott is the President and CEO of the Jewish Home Family, located in northern New Jersey. Serving older adults for more than a century, the Jewish Home continuum of services includes the Jewish Home at Rockleigh, offering both long term care and short term rehabilitation; Jewish Home Assisted Living; Jewish Home at Home, providing home health care and adult medical day services; Jewish Home Foundation. Prior to joining the Jewish Home Family in 2014, Silver Elliott served as the President and CEO of Cedar Village Retirement Community in Cincinnati, Ohio.
Silver Elliott began her career in acute care, serving strategic roles in marketing, planning and public relations. Entering the world of older adult services in 2007, she immediately became involved on both a State and national level. She serves on the board of the Association of Jewish Aging Services and served as chair of the board from 2013-2015. She was a board member of LeadingAge Ohio and joined the board of LeadingAge New Jersey in 2015. She is also a member of the board of the VNA of Central New Jersey. In 2012, Silver Elliott joined the board of LeadingAge, the organization that represents nonprofit senior care providers around the country and currently serves as chair elect.
Passionate about elder abuse prevention, Silver Elliott was instrumental in founding the Shalom Center at Cedar Village in 2010 and in developing SeniorHaven at the Jewish Home in 2015. She is the recipient of numerous State and national awards for innovation and leadership and has spoken at conferences both around the country and internationally. Danielle Pierotti is the Vice President of Quality and Performance Improvement with the Visiting Nurse Associations of America. In partnership with member home health and hospice organizations across the country, she brings innovation to care practices, seeking to continuously improve practice and meet evolving patient needs through the application of evidence.
Danielle has been a nurse for more than 20 years, practicing in hospitals, ambulatory and home-based care and serving in medical-surgical, oncology, quality and leadership roles.
Prior to joining VNAA, she led hospice services with the largest provider in Iowa, ensuring end-of-life care across 42, mainly rural, counties. She developed and initiated a new specialty role in hospital-based nursing quality guiding the organization to achieve Magnet status. Her doctoral research was an examination of nurse-sensitive outcomes in hospitalized oncology patients.

Erin Denholm is the President and CEO of Trinity Home Health Services in Livonia, Michigan. Supporting home based programs in 21 states, Ms. Denholm is leading one of the top 10 largest home care organizations in the country. Previously Denholm served for 20 years in Centura Health most recently as SVP of Clinical Transformation for Centura Health. A Robert Wood Johnson Nurse Executive Fellow, she also served as one of 72 CMS Innovation Center Advisory Council members. She is a distinguished leader in healthcare, noted for her visionary and passionate commitment to transcend healthcare beyond hospital walls, innovate new models of care that improve quality while decreasing costs, increase healthcare literacy among consumers and, in particular, empower consumers with the resources to direct their care. She pioneered the use of telehealth technology in reducing emergency room visits and adverse outcomes among heart failure patients in Colorado, and led the successful legislative effort to make Colorado the first state in the country to provide Medicaid funding for telehealth services.
Ms. Denholm has won numerous accolades for her work. Under her auspices, Centura Health at Home had been recognized as a world-class workplace by the Gallup organization. She is a recipient of The Denver Business Journal Champion in Healthcare award and her collaborative spirit inspired a joint venture with The Children's Hospital to fund and operate the first palliative care program in Colorado designed exclusively for children and their families which has received national acclaim. She is an international speaker and consultant on telehealth nursing and post -acute care leadership. Most recently Ms. Denholm co-authored a chapter in Nursing Policy and Politics, which won the 2015 ANA book of the year. Ms. Denholm holds an MSN from Regis University and a BSN from The College of Mt. St. Joseph. She currently serves on the boards of the Colorado Center for Nursing Excellence. She Historically she has been on the Home Care Association of Colorado Board of Directors, The Colorado Hospice and Palliative Care Organization and has served on the Board of Directors for the Rocky Mountain Children's Choir.

Esther Emard recently retired from her role as the Chief Operating Officer of NCQA after 12 years, and continued to serve as a consultant to the organization for an addition year. In her role as COO she was responsible for all aspects of business planning and operations in supporting of the organization's strategic plan. She also served as Chair of the Leadership Team and Management Team for the organization.
Prior to assuming her position with NCQA, Emard was the Senior Vice President of Regional Operations for Harvard Pilgrim Health Care, where she was responsible for all regional operations in the states of Maine, Vermont, New Hampshire, Rhode Island and Massachusetts. She also served as President of Harvard Pilgrim Health Care of New England, the Rhode Island affiliate of Harvard Pilgrim Health Care.
She has held additional positions as the Associate Director of Nursing for Morton Hospital and Medical Centers, Inc. of Taunton, Massachusetts.
Emard is currently a member of the National Honor Society in Nursing and Member of the Women Business Leaders of the U.S. Health Care Industry Foundation.
Emard was recently a member of Medical Reserve Core (MRC), serving as an Incident Coordinator, for Fairfax County, State of Virginia. She served for several years as both a RN and an Incident Coordinator. She held an appointment by the Director of Fairfax Health Department to the County's Leadership Council and as the MRC representative to the Citizens' Core Council.
Emard is currently an Instructor for The George Washington University School of Nursing. She is also the Routhier Visiting Clinical Professor, 2014-2015 for the University of Rhode Island College of Nursing and an Assistant Professor teaching at the graduate level. Additionally, she has been a consultant to AACN for the past 3 years engaged by the QSEN Initiative (Quality and Safety Education in Nursing) to be one of three faculty holding workshops across the United States on the new Master and Doctoral level competencies.
Emard is currently a member of the Board of Directors for the Care New England Health Care System. In that capacity serves as the Vice Chair of the system's Quality & Credentials Committee and Co-Chair for the systems' VNA Quality and Clinical Services Committee.
Emard received a Masters' degrees in Nursing from the University of Rhode Island (URI) and a second Masters' degree from URI in Labor and Industrial Relations. Faith Scott has worked in a leadership role in the Home Health industry for more than thirty years. She successfully built Visiting Nurse Association of Northern New Jersey, started a private duty company, and started a certified hospice. In addition, she has grown community based services under several grant programs including an Adult Day Care Center, to sustain elderly living in the community. Scott has worked to identify data to use as key analytics to build strong metrics to drive business. She is strong in strategic planning and adopts innovative business solutions to sharpen organizational performance.
More importantly, Scott advocates for patient free choice and the right for Visiting Nurse Associations to compete in the new marketplace as health care reform evolves and post acute care needs emerge. She advocates for all patients to have access to services which will enable them to live freely and with dignity to the end of their lives, in their own homes. Additionally, she advocates for nurses to excel to a heightened level of professionalism, where their voice and accountability are at the planning table to shape a model of health care in the United States. Lastly, she advocates that as American tax payers feel the pressure to constrain Medicare spending the application of home health is viewed more widely as a solution which also enhances quality of life.

Gary attended and graduated from Boston University with a Bachelor of Arts Degree. Continuing his education, Gary received his Masters Degree in Public Administration from American University.
While attending graduate school, Gary became a program coordinator for the National Association of County Health Officers. This began his career in health care services. He initiated the National Technical Assistance Program for County Health Officers that contained alcohol, drug, and mental health programs. Subsequent to this project, Gary became Senior Project Director for the Group Health Association of America (GHAA). Here he innovated substance abuse and mental health outpatient programs in HMO settings. Later, under federal contract, Gary provided technical assistance to HMOs on a national basis for these programs.
Gary became the Executive Director of Broward Community Health Plan that became Health Care of Broward, Broward County's first federally qualified HMO. The company was sold to Health America Corporation and Mr. Jacobs assumed the role of Senior Vice President of Development. HAC became the first national HMO management corporation. Gary left HAC to start Health Management, Inc. (HMI), a managed care consulting and Management Company. Under his direction, the company went public while pioneering innovative health care programs. He acquired the Staff Builders Health Care Services franchise of Broward County, Florida and grew it to a $15M operation.
Gary formed Capitated Health Care Services, Inc., a management company specializing in long-term care systems. The company was sold to Universal American Financial Corporation (UAM). Gary served as President of CHCS Services, Inc., a third party administrator focusing on senior health insurance and non-insurance products.
Gary was also given the title of Senior Vice President Corporate Development for UAM. In his capacity as Senior Vice President Corporate Development and Strategic Planning for Universal American, (UAM) he was responsible for implementing enterprise wide planning initiatives, strategic alliances, government affairs, and identification of acquisitions and new business opportunities. During his tenure UAM completed over 12 acquisitions of health insurance and health care organizations. His focus was on identifying trends through Federal and State policy initiatives and targeting companies that possessed the core competencies to expedite UAM's entry into that market.
Mr. Jacobs is a frequent speaker on Health policy issues and Health reform issues with a particular emphasis on Medicare, Medicaid, Dual Eligibles and Managed Long Term Care issues. Ian Juliano has significant experience in the Healthcare IT and data analytics sectors. Prior to Trella Health (formerly Excel Health) he was CEO of HomecareCRM (HCRM), a census growth platform for the Post-Acute (PAC) industry, with clients that included Amedisys, VITAS, Golden Living, Consulate and HCR ManorCare. During his tenure, Ian led HCRM to a doubling of recurring revenue within five quarters, and engineered the successful sale of HCRM to SigmaCare, a VC-backed PAC IT provider. Prior to HCRM he was EVP of Corporate Development for Brightree, the largest software provider to the Durable Medical Equipment market. At Brightree he led the acquisition of CareAnyware, a SaaS-based Home Health and Hospice EMR, to expand Brightree's reach into those markets. Prior to Brightree Mr. Juliano was CFO of CRE Secure, a VC-backed e-commerce platform, which was acquired by ControlScan.
Ian also spent over 15 years in senior Corporate Development roles, including Certegy (now Fidelity National Finance) and West Corporation, where he led over $1 billion in M&A bids, closing nearly half, in the Healthcare IT and Fin Tech industries. His early career began at Price Waterhouse.
Through the University of Pennsylvania Dual Degree Program, Ian earned Bachelor degrees in Business and Psychology from the Wharton School and the College of Arts and Sciences, respectively. He has an MBA from the Kellogg Graduate School of Management and earned academic honors at both institutions.

Kathy Peirce, RN, BSN, MS, has been a registered nurse for more than 30 years. Her experience includes acute care and 20 years of Home Health and Hospice care. Kathy joined the VNA of Care New England in 2014 after working for Hartford HealthCare and Masonicare Home Health and Hospice in Connecticut, serving as Chief Operating Officer. She is a graduate of Saint Anselm College in Manchester, NH, and Rensselaer at Hartford, in Hartford, CT, where she earned a Master of Science in Management. Kathy has served on several community boards and committees. Currently, she is serving on the board of Rhode Island Partnership for Home Care and chairs their education committee. Kathy is originally from Rhode Island.

Katie Smith Sloan is president and CEO of VNAA/ElevatingHOME and LeadingAge. Guided by the mission to be the trusted voice for aging, Sloan advances the organization's strategic priorities to increase impact through advocacy, enhanced member value and generates ideas to improve our current system of services and supports. As LeadingAge's President and CEO, Katie Smith Sloan seeks common ground with other stakeholders to address ageism and to promote innovation.

Keith Crownover, CEO of Delta Health Technologies, has more than 33 years of experience in home health, hospice, and private duty home care information technology. He was an active participant in four key industry projects: 2012 State of the Industry Study, 2011 Delta Study to Reduce Hospitalizations, 2010 Delta National Excellence in Therapy Project, and 2009 Delta OASIS-C Best Practices Project.
His career with Delta began in 1982 in the Computer Operations department. Since that time, he has held a number of different positions with the company in implementation, support, sales and sales management. Keith's extensive experience in the healthcare industry is a substantial asset to Delta and the customers it serves. Memberships in technology and industry panels emphasize his commitment to partnerships between industry and research. Lynda Tanner is the President and CEO of VNA Health, an organization of more than 370 employees providing hospice, home health, palliative and personal care service programs to more than 12,000 people annually across Santa Barbara County.
After successfully pressing for legislation to increase end-of-life inpatient beds, in 2011 Ms. Tanner oversaw the organization's opening of a new state-of-the-art inpatient hospice facility, Serenity House. She also directed the acquisition of Community Home Health which increased VNA Health's delivery of home health services by 35%, and successfully obtained branch status for a satellite location in Santa Ynez. In 2012, Ms. Tanner oversaw a move to a new location in downtown Santa Barbara with more space to house the growing organization. Since 2014, Ms. Tanner has participated in the organization's annual PHorum: Perspectives in Healthcare, providing leadership on key healthcare priorities in Santa Barbara. In late 2024, after five years of diligent work, Ms. Tanner announced that VNA Health had achieved Level 4 of the We Honor Veterans, a program of the National Hospice and Palliative Care Organization in collaboration with the Department of Veterans Affairs. VNA Health is the only hospice organization in the Tri-County area (Santa Barbara, Ventura, and San Luis Obispo) to have achieved this level.
Ms. Tanner is active in many professional, service, business and community organizations and currently serves as: Chair of the PAPA Committee on the Board of Directors for California Association for Health Services at Home (CAHSAH); Finance Officer on the Board of Directors for CenCal Health; Member of the Council of Leaders for the Alliance for Living and Dying Well; and Chair of the Audit/Compliance Committee for the Board of Directors for Casa Dorinda. M. Kate Rolf is a senior healthcare executive with over 15 years of leadership experience, which is complemented by her master in business administration, certification as a Home Care Executive, fellowship from the American College of Healthcare Executives, and licensure as a New York State Nursing Home Administrator.

Marcia P. Reissig has been the Chief Executive Officer of Sutter Care at Home - Sutter Health's home care affiliate - since March 2007. Ms. Reissig is responsible for the development and administration of the system's $300 million home care program, which includes home health, hospice, home infusion pharmacy, home medical equipment and respiratory therapy, private services, and flu and wellness services. Sutter Care at Home's nearly 2,000 employees serve over 19,000 patients a day throughout northern California.
A nurse, with more than 3 decades of management and clinical experience, Marcia has dedicated much of her career to providing vision and leadership in the integration of home care organizations with each other and across the continuum of care. She held chief executive positions at Partners Health Care in Boston and two other large integrated health care systems in New York prior to joining Sutter Health. She serves on Executive Committees of the Alliance for Home Health Quality & Innovation (AHHQI) Board of Directors and the LINC group of system-affiliated post-acute and community-based services providers. Past board experience includes the National Association for Home Care & Hospice (NAHC) Board of Directors, the Hospital Home Care Association of America, and the boards of the New York, Massachusetts, and California home care associations. A frequent presenter nationally, Marcia has earned a number of awards and honors including the Mass Technology Leadership Award in recognition of her leadership role in utilizing technology to manage a full spectrum of clinical services. Dr. Landers is the President and CEO of the Visiting Nurse Association Health Group, Inc., the nation's second largest not-for- profit home health care organization. As a certified family doctor and geriatrician, Dr. Landers places a strong emphasis on house calls to the vulnerable elderly and has a specialized interest in geriatric medicine, home health, hospice and palliative care.
Dr. Landers is a graduate of the Case Western Reserve University School of Medicine and the Johns Hopkins University School of Hygiene and Public Health. Sue joined the Transpirus team in January of 2016 as the Senior Vice President of Clinical Services. In her role, she is responsible for leading the clinical consulting services division including HH/Hospice coding, quality assurance medical review audit, operations consulting and regulatory compliance. Sue will be responsible for clinical operations and for identifying clinical business development options to meet client needs.
Sue has over 20 years in home care operations and strategic leadership. Prior to her role at Transpirus, Sue was the Vice President of Home Health at Advanced Home Care and at Life Care at Home, both of which are multi-state home care providers. Previously, she was the Administrative Director at Bon Secours Home Care which is a multi-site hospital based home care agency. Terri Cunliffe serves as Chief Executive Officer of Covenant Retirement Communities, the nation's fifth largest not-for-profit senior services provider. Ms. Cunliffe, who has nearly 30 years of senior living experience, has served in a variety of executive leadership roles at CRC for the past 27 years. In these roles, she has implemented wellness and corporate initiatives that have effected change across the organization's 14 retirement communities serving approximately 5,000-seniors nationwide.
As CRC's Vice President of Health & Wellness, she created and executed the company's LifeConnect® approach to whole-person wellness; where CRC staff connects residents with resources and opportunities to help them achieve wellness in every dimension of life, including spiritual, emotional, social, community, vocational, physical, intellectual and residents/staff involvement.
Ms. Cunliffe's roots with CRC are deep, dating back to January 1988, she moved to Covenant of Florida where she opened the community's assisted living facility; in 1991 she became the Healthcare Administrator and five years later was named Executive Director.
Ms. Cunliffe was chair of LeadingAge Florida, formerly known as Florida Association of Homes and Services for the Aging, from 2009-2011. She was named FAHSA Executive of the Year in 1999 and received its chairman's award in 2005.
Ms. Cunliffe holds a Bachelor of Science degree in Long-Term Care Administration from the University of Minnesota, Minneapolis, Minn. and a master's degree in health services administration from Nova Southeastern University, Fort Lauderdale, Fla. She is licensed nursing home administrator in the state of Florida.

Tracey Moorhead is President and Chief Executive Officer of the Visiting Nurse Associations of America (VNAA). VNAA advances quality, value and innovation in home-based care. Tracey spearheads VNAA's strategic vision and growth, develops transformational care delivery and payment models and leads policy formulation before the media, allied organizations and constituents, and Federal policy makers. Tracey brings a population health management approach to home-based care having previously served as CEO of the Care Continuum Alliance/Population Health Alliance. CCA/PHA advances wellness, disease management and care coordination models for payers and purchasers through population health management frameworks, best practices, measure sets and methodologies.
