Amber Elswick joined Capstone Health in June 2014. As the Operations Director, Mrs. Elswick oversees the development and implementation of operational strategies and tactics, business plans and goals, corporate compliance program, community outreach, and vendor relations.
Prior to joining Capstone Health, Mrs. Elswick served as the Program Coordinator for AdvanceMed Corporation, Medicaid Integrity Contractor for the Medicaid Integrity Program Division of the Centers for Medicaid & Medicare Services. While in this capacity, Mrs. Elswick served as a senior advisor to the Program Director and was responsible for the direction of policy development and project management of federal projects to sustain quality control between the Centers for Medicaid & Medicare Services Government Project Office and AdvanceMed Corporation. Additionally, Mrs. Elswick assisted in analyzing claims data to identify high risk areas and potential vulnerabilities to combat fraud, waste, and abuse for the Chicago, Dallas, and San Francisco Regions.
Mrs. Elswick holds a Bachelor of Business Administration degree from Belmont University with a concentration in Audio Engineering Technology & Production and Vocal Performance. Mrs. Elswick is a member of Walker County Health Action Partnership, Winston County Diabetes Coalition, Sigma Alpha Iota International Music Fraternity, the Health Care Compliance Association, and the Association of Certified Fraud Examiners.
Mrs. Elswick resides in Jasper, Alabama with her husband and dog, Bella. She is actively involved at her church and enjoys gardening and traveling.
Amy Causey joined Capstone Health in July 2023. As a Board-Certified Family Nurse Practitioner and certified Neonatal Resuscitation Program Provider, Mrs. Causey serves both adult and pediatric patients at the Summit and Parrish locations.
Prior to joining Capstone Health, Mrs. Causey served on the Neonatal Intensive Care Unit at UAB and the Coronavirus Testing Center at UAB Highlands with clinical experience at Kirklin Clinic PACT Anesthesia Clinic, Dabbs and Hyland Pediatrics, and Ascension Women's Health at St. Vincent's Birmingham.
Mrs. Causey obtained a Bachelor of Science and Master of Science in Nursing from the University of Alabama in Birmingham. Mrs. Causey is a member of Sigma Theta Tau Honor Society of Nursing and the American Nurse Credentialing Center.
Mrs. Causey resides in Walker County, Alabama with her husband and daughter.
Angela McNutt joined Capstone Health in November 2019 as a Certified Family Nurse Practitioner. With the expansion of Capstone Health to Winston County, Alabama, Mrs. McNutt assumed the role as the primary Medical Provider at the Double Springs location.
Prior to joining Capstone Health, Mrs. McNutt served at Northwest Medical Center, Lakeland Community Hospital, Walker Baptist Medical Center, Family Care First, and Burdick-West Hospital. Mrs. McNutt has over 20 years of experience in Family Practice, Intermediate Care Unit, Intensive Care Unit, Medical-Surgical, and Emergency Room Care.
Mrs. McNutt obtained an Associate of Applied Science in Nursing degree from Northwest Shoals Community College and Master of Science in Nursing degree from the University of Alabama in Huntsville. Mrs. McNutt is a member of the American Academy of Nurse Practitioners.
As a native of Winston County, Mrs. McNutt is grateful to have the opportunity to care for her community. She resides in Haleyville, Alabama with her husband and three sons.
April Poe joined Capstone Health in February 2024. As a Certified Family Nurse Practitioner, Mrs. Poe serves patients at the West Jasper location.
Prior to joining Capstone Health, Ms. Poe served at Simon Williamson Complete Health, Medical Center Barbour Family Care Clinic, and Princeton Hospital, and as a Clinical Instructor for the Bevill State Nursing program. Ms. Poe has several years of experience in Family Practice, Medical Intensive Care, Post-Op Recovery, and Palliative Care.
Ms. Poe obtained a Bachelor of Science in Nursing degree from the University of Alabama and Masters of Science in Nursing degree from the University of Alabama in Birmingham. Ms. Poe also holds a Bachelor of Science in Sociology degree from the University of North Alabama. Ms. Poe is a member of the American Academy of Nurse Practitioners.
Ms. Poe resides in Walker County with her family.
Dr. Brandi J. Rudolph-Bolling joined Capstone Health in March 2018.As a Triple Board-Certified Psychiatrist, Child and Adolescent Psychiatrist, and Pediatrician, Dr. Rudolph-Bolling serves patients via telehealth at the Parrish location.
Dr. Rudolph-Bolling works in a variety of medical settings and brings a myriad of knowledge and experience in treating all mental health issues. Dr. Rudolph-Bolling serves both urban and rural areas in adult and child inpatient, outpatient, telehealth, and medical settings with special interests including autism spectrum disorders; childhood obesity and its relation to and effect on mood; bullying; and attention deficit hyperactivity disorder (ADHD). She believes in the careful integration of the fields of neuroscience, psychology, psychiatry, child development, and education to develop treatments to meet biological, psychological, and social needs.
Dr. Rudolph-Bolling was educated in Birmingham City Schools and obtained a Bachelor of Science in Neuroscience degree from Vanderbilt University and Doctorate of Medicine from Meharry Medical College. Dr. Rudolph-Bolling completed a Triple Board Residency Program at Indiana University School of Medicine in Indianapolis, Indiana.
Dr. Rudolph-Bolling resides in Jefferson County, Alabama with her husband and two toddler sons. In her spare time, she enjoys working with her husband to mentor children in their community and spending time with her family.
Bryan Miller joined Capstone Health in June 2023. As a Licensed Masters Social Worker, Mr. Miller serves patients at Capstone's Viking Health School-Based Health Center. Prior to joining Capstone Health, Mr. Miller served as a Drug Court Counselor, a Group Facilitator and Certified Trauma Interventionist with Youth Advocate Programs, and a S.A.I.L Specialist for victims of domestic violence.
Mr. Miller obtained a Bachelor of Social Worker degree and a Master of Social Work degree from the University of Alabama.

Chris Shariett joined Capstone Health in June of 2022. As a Certified Family Nurse Practitioner, Mr. Shariett serves patients at the Parrish location.

Connie Oden joined Capstone Health in June 2017. As a Licensed Independent Clinical Social Worker, Ms. Oden serves patients via telehealth at the Jasper Summit location.
Prior to joining Capstone Health, Ms. Oden served at Eastside Mental Health Center, Unity Hospice, Children's Hospital, Brookwood Hospital, Northwest Alabama Mental Health, Cullman Regional Medical Center, and Hill Crest Hospital. Ms. Oden has over 15 years of experience in individual counseling and group therapy and over 20 years of experience in nursing as a Clinical Director, Teacher, and Group Leader in a partial-hospital setting and various in-patient settings.
Ms. Oden obtained an Associate in Nursing degree from Calhoun Community College, a Bachelor of Science in Psychology degree emphasizing in Biology from the University of Alabama, a Master of Arts in Education degree from the University of Alabama in Birmingham, and a Master of Social Worker degree from the University of Alabama. Ms. Oden attained her Licensed Masters Social Worker Certification in 1998 and Licensed Independent Clinical Social Worker Certification and Private Independent Practice license in 2000.
Ms. Oden is a member of the Alabama Clinical Social Work Society, Birmingham Bluestocking Book Group, and Mindfulness-Based Cognitive Therapy. She resides in Trussville, Alabama and enjoys writing, reading, hiking, and spending time with her family.

Crystal Bentley joined Capstone Health in February 2010. As the Nursing Director, Mrs. Bentley provides clinical oversight to all nursing staff, development of quality improvement efforts, coordination of Patient Centered Medical Home (PCMH) recognition, and emergency preparedness disaster and risk management programs.
Prior to joining Capstone Health, Mrs. Bentley began her medical career in 1996 at Cullman Health Care and spent twelve years working in the Obstetrics & Gynecology field.
Mrs. Bentley obtained an Associate in Nursing degree from Bevill State Community College. She resides in Curry, Alabama with her husband. In her spare time, you may find her spending time with her family or cheering on the Alabama Crimson Tide.
Prior to joining Capstone Health, Mrs. Atmore served at Trinity Hospital as a Diabetes Educator Program Coordinator and preceptorships at UAB Comp Clinic and Kirklin Clinic in Obstetrics and Gynecology, St. Vincent's Hospital in Pediatrics, and the Diabetes & Lipid Center in Internal Medicine. Mrs. Atmore uses her skill in assessing and evaluating each patient based on individual requirements, implementing solutions and education to emphasize preventative health, and providing diabetes self-management tools.
Mrs. Atmore obtained a Bachelor of Arts in Criminal Justice and both a Bachelor and Master of Science in Nursing degree from the University of Alabama in Birmingham. Mrs. Atmore is a member of the American Association of Nurse Practitioners, American Nurses Credentialing Center, Alabama Association of Diabetes Educators, and volunteers for Tot Shot Organization.

Dr. Abby Chambers joined Capstone Health in December 2023. Dr. Chambers serves as the primary Pharmacist for the Capstone Pharmacy at the Nauvoo location. Prior to joining Capstone Health, Dr. Chambers served at Gateway Family Pharmacy, English Plaza Pharmacy, and Harrison Drug.
Dr. Chambers obtained both an Associate in Science degree in Pre-pharmacy Studies from Bevill State Community College, Bachelor of Science in Pharmacy Studies from Samford University, and Doctorate of Pharmacy from Samford University McWhorter School of Pharmacy.
Dr. Chambers is a native of Winston County, Alabama. In her spare time, she enjoys spending time with family.

Dr. Emilee Powell joined Capstone Health in April 2024. Dr. Powell serves as the primary Pharmacist for the Capstone Pharmacy at the Parrish location. Prior to joining Capstone Health, Dr. Powell served at Publix Pharmacy and Rite-Aid Pharmacy.
Dr. Powell brings several years of advanced pharmacy practice experience in community pharmacy and managed care. During her membership with the American Pharmacist Association, she led a team in conducting numerous health screenings within her local community.
Dr. Powell holds a Bachelor of Science degree in Nutrition and Wellness from the University of Montevallo, as well as a Bachelor of Science degree in Pharmacy Studies and a Doctorate of Pharmacy from Samford University's McWhorter School of Pharmacy.
Dr. Powell resides in the Birmingham area with her family.

Dr. F. David Jones has served as the Executive Director for Capstone Health since 2011. With over 25 years of healthcare experience, Dr. Jones contributes his dedication and passion for improving quality and affordable healthcare to vulnerable and rural populations due to his hands-on experience as a Registered Nurse, Nurse Practitioner, and Health Care Executive.
Dr. Jones obtained both a Bachelor and Master in Nursing degree from Troy University and Doctorate of Nursing Practice from the University of Alabama. Dr. Jones is also a 2008 Graduate of the UCLA Health Care Executive Program and holds a Family Nurse Practitioner Certification from the American Nursing Credentialing Center. Dr. Jones is an Adjunct Instructor of Nursing, a member of Sigma Theta Tau International Nursing Honor Society, the Jasper Kiwanis Club, and serves on various Board of Directors and Community Committees.
Dr. Jones resides in Walker County and serves at a local church. In his free time, he enjoys gardening, traveling, kayaking and spending time with his wife and three daughters.
Dr. Jaclyn Garner joined Capstone Health in September 2019. Dr. Garner served as the primary Pharmacist for the Capstone Pharmacy at the Nauvoo location prior to transitioning to the primary Pharmacist role for the Capstone Pharmacy at the Summit location.
Prior to joining Capstone Health, Dr. Garner served at Parkway Discount Drugs, Lunar Pharmaceuticals, and Pharmacy Link, Inc. with advanced pharmacy practice experience in General and Pediatric Medicine, Ambulatory Care, and Nuclear Pharmacy.
Dr. Garner obtained both an Associate of Arts degree and Associate of Science degree from Jefferson State Community College, Bachelor of Science in Pharmacy Studies from Samford University, and Doctorate of Pharmacy from Samford University McWhorter School of Pharmacy. Dr. Garner is a member of the American Pharmacists Association.
Dr. Garner has a passion for serving in underserved communities and a willingness to help others. In her spare time, she enjoys cooking, spending time with her husband and family, and visiting Disney World.

Dr. Burroughs is a graduate of Walker High School and obtained a Doctorate of Pharmacy from Samford University McWhorter School of Pharmacy. Dr. Burroughs brings a love and servanthood for helping others become as healthy as possible.
Dr. Lauren Burroughs joined Capstone Health in October of 2015. With the expansion of pharmacy services at Capstone Health in 2016, Dr. Burroughs assumed the primary Pharmacist role for Capstone Pharmacy at the Nauvoo location further transitioning to the Pharmacist role for the Capstone Pharmacy at the Parrish location and now as the Pharmacy Director.
As the Pharmacy Director, Dr. Burroughs oversees all pharmacy services, prescription fills, education of medications, disease states and medical equipment, and medication therapy for Capstone Health.
Dr. Marlene Howell joined Capstone Health in October 2019. As a Board-Certified Family Nurse Practitioner, Dr. Howell serves patients at the Jasper Summit location.
Prior to joining Capstone Health, Dr. Howell served at American Family Care, Brookwood Baptist Medical Center, Milan General Hospital, and Jackson-Madison County General Hospital. Dr. Howell has over 15 years of experience in Surgical Intensive Care and Post-Anesthesia Care.
Dr. Howell obtained an Associate in Nursing degree from Jackson State Community College, a Bachelor of Science in Nursing degree from Troy University, and a Doctorate of Nursing Practice from Troy University. Dr. Howell is a member of Sigma Theta Tau International Nursing Honor Society, American Academy of Nurse Practitioners, Central Alabama Nurse Practitioner Association, and West Alabama Nurse Practitioner Association.

Dr. Tandy Hoover joined Capstone Health in December 2023. As a Board-Certified Family Nurse Practitioner, Dr. Hoover serves both adult and pediatric patients at Capstone's Hilltop Health School-Based Health Center. In addition, Dr. Hoover serves as a Clinical Instructor for the UAB School of Nursing program.
Prior to joining Capstone Health, Dr. Hoover served at the Kirklin Clinic PACT Anesthesia Clinic at UAB, Pulmonary Med-Surgical and Endoscopy Units at UAB, and Family Medical Associates West.
Mrs. Causey obtained an Associate of Science in Nursing from Wallace State Community College and Associate in Science of Business Administration from Bevill State Community College, a Bachelor of Science in Nursing from the University of Alabama in Huntsville, and a Master of Science in Nursing and Doctorate in Nursing from the University of Alabama in Birmingham. Dr. Hoover is a member of the American Nurse Credentialing Center.

Heather Nunnelley joined Capstone Health in November 2020. As a Certified Family Nurse Practitioner, Mrs. Nunnelley serves children and adults at Capstone's Viking Health School-Based Health Center.
Prior to joining Capstone Health, Mrs. Nunnelley served at DCH Northport Medical Center, UAB Hospital, and preceptorships at Walker Medical Diagnostics and FMC Medical Clinic. Mrs. Nunnelley has several years of experience in Intensive Care, Neonatal Intensive Care, and Continuing Care Nursery.
Mrs. Nunnelley obtained a Bachelor of Science in Nursing degree from Mississippi University of Women and Master of Science in Nursing degree from the University of Alabama in Birmingham. Mrs. Nunnelley is a member of the American Association of Nurse Practitioners, Sigma Theta Tau International Honor Society of Nursing, Phi Theta Kappa Honor Society, and Phi Kappa Phi.

Kaydian Smoot joined Capstone Health in August 2024. As a Licensed Masters Social Worker, Ms. Smoot serves patients at Capstone's Hilltop Health School-Based Health Center.
Prior to joining Capstone Health, Ms. Smoot served as a School-Based Mental Health Therapist for Walker County Schools and as a Mental Health Therapist at Quality Correctional Health Care. Ms. Smoot has several years of experience in individual counseling, family counseling, and group therapy specializing in prevention, intervention, and postvention for crisis situations.

Lindsey Jones joined Capstone Health in August 2009 as a Certified Family Nurse Practitioner. With the expansion of Capstone Health in January 2015, Mrs. Jones transitioned from the Parrish location and assumed the role as the primary Medical Provider for the Jasper Cottage location. In September 2022, the Jasper Cottage location moved to the Capstone Summit location where Mrs. Jones now sees patients.
In addition to her Nurse Practitioner duties, Mrs. Jones serves at the Clinical Director for Capstone Health. As the Clinical Director, Mrs. Jones provides oversight to all clinical support services, accurate and timely diagnosis, delivery of relevant, high-quality medical care and treatment, and monitoring of quality improvement program and initiatives. Mrs. Jones has over 20 years of experience working in Medical-Surgical, Emergency Room Care, Urgent Care, and Family Medicine.
Mrs. Jones obtained an Associate in Nursing degree from Bevill State Community College and a Master of Science in Nursing degree from Samford University. Mrs. Jones is a member of Sigma Theta Tau International Nursing Honor Society and American Nurse Credentialing Center.

Maddie Noe joined Capstone Health in August 2023 with several years of experience in Family Practice, Medical-Surgical, and Post-Op Recovery. As a Certified Family Nurse Practitioner, Ms. Noe serves as the primary Medical Provider at the Nauvoo location.
Prior to joining Capstone Health, Ms. Noe served at Divine Family Wellness, Kelly Urgent Care, North Mississippi Medical Center, Brookwood Medical Center and as a Clinical Instructor at North Mississippi Medical Center in Hamilton as part of the Bevill State Nursing program.
Ms. Noe obtained a Bachelor of Science in Nursing degree from the University of Alabama in Birmingham, a Masters of Science in Nursing-Population Health degree with concentration in Emergency Management from Jacksonville State University, and a Post Masters Nurse Practitioner Certificate from University of North Alabama.
