BARBARA A REISER

Our agency offers case management to individuals who are in need of assistance with coordinating medical care, mental health care, and/or daily living functions. This services includes continually assessing the client's current situation, determining and resolving unmet needs, and helping attain client defined goals for improvement in medical, emotional and psycho-social status. This is done by the case manager through an interdisciplinary approach, working with other providers and community agencies to ensure continued access to this program, and other programs that may be of service to the individual.
